What Is an Air Force Form 1181?

The Air Force Form 1181, Child Care Registration Form, is a required document for all children enrolled in military-affiliated childcare programs, including Family Child Care (FCC) homes. This form collects essential information about each child, such as:

  • Parent/guardian contact information
  • Emergency contacts
  • Medical conditions or allergies
  • Authorized pick-up individuals


The 1181 ensures your provider has accurate, up-to-date information to keep your child safe and properly cared for. It must be completed before your child begins care and updated whenever information changes.

Paints, crayons, play-dough, glue, scissors, markers, craft kits, themed activity materials, and seasonal projects — fully provided.

🍽️ Meals & Snacks

Nutritional breakfasts, lunches, and snacks (per USDA guidelines) — no need to pack food!

**Breast Milk & Formula Parents Provide!**

🍼 Baby Care Supplies (If applicable)

Highchairs, bibs, infant learning toys, baby-safe equipment, and all developmental materials for ages 0–24 months.

🛏️ Nap Essentials

Cots/nap mats, individual clean blankets (or parent-provided), quiet-time activities, and a calming sleep environment.

🧼 Hygiene & Cleaning Supplies

Sanitized play spaces, handwashing materials, tissues, child-safe cleaning products, and a daily cleaning routine.

🏡 Outdoor Play & Excursions

Riding toys, outdoor sensory activities, chalk, balls, and movement-based learning tools. Carseats provided for field trips!

Immunizations + Current Season's Flu

12.8. Immunizations. Children and youth enrolled are required to follow the Advisory Committee on Immunization Practices/Center for Disease Control and Prevention recommended age-appropriate immunizations and outlined in DoDI 6060.02 and DoDI 6060.04.


12.8.4. All children and staff are required to have the annual flu vaccine no later than 30 calendar days after enrollment. (T-0) Note: Vaccines are required prior to the flu season. Documentation of the vaccine is required no later than 30 November each year. (T-1)
